Registrations
If you have been nominated by your Junior Football club, complete the below Registration Form to join our program in the new year.
As part of the registration process you will need to sign a permission and medical information form.
You can do this by;
1) downloading the medical and permissions form below
2) save as your childs name
3) complete the form, save again
4) drag and drop/uploading into the registration form below.
(Tip: Use your smart phone or computer to do all the above & never print another form again)
You will also need to read and agree to the Players and Parents Code of Conduct, found at the bottom of this page.

CODES OF CONDUCT
PEAK Football supports the Codes of Conduct as introduced by the AFL and AFL Victoria.
PARTICIPANTS
• Attend a minimum of 4 PEAK Football training sessions
• Attend sessions in full uniform or otherwise be excluded
• Dress with pride and support the PEAK colours and apparel, however the PEAK uniforms are not to be worn at Junior Club training sessions/ game days
• Participate in all PEAK Football community engagements, including the selling of PEAK raffle tickets
• Enjoy the Game and take pride in representing PEAK Football
• Play by the rules – the rules of PEAK Football and the laws of the game
• Never argue with an umpire or other official – without these people, you can’t play football
• Control your temper – verbal abuse of officials and sledging other players doesn’t help you enjoy your football
• Be a team player – it’s a team game, treat it that way
• Treat all players as you would like to be treated- fairly
• Co-operate with your coach, the umpires and team-mates
• Play for your own enjoyment and to improve your skills
• Don’t use ugly remarks or make statements in public (print or electronic media) based on race, religion, gender or ability – you’ll let down your coach, team-mates and family if you do and many such comments can be legally contested
• Attend training sessions with your Journal completed or otherwise be excluded
• Any behaviour that may bring the PEAK Football name into disrepute will see my position within the Academy revoked.
PARENTS
• Encourage your child to participate in all PEAK Football community engagements, including providing assistance to sell PEAK raffle tickets
• Remember that you are there for the participants to enjoy the game of Football
• Encourage participation, but don’t force it
• Teach that enjoyment is more important than winning
• Never ridicule mistakes or losses – supporters are there to support not downgrade
• Lead by example and respect all players, coaches, umpires and spectators – physical or verbal abuse will not be tolerated
• Recognise all volunteers who are giving up their valuable time
• Never publicly criticise umpires – raise personal concerns with club officials in private
• Don’t use ugly remarks or make statements in public (print or electronic media) based on race, religion, gender or ability – many such comments can be legally contested
• Any behaviour that may bring the PEAK Football name into disrepute will see my child’s position within the Academy revoked.
By registering with PEAK Football we agree to be bound by these Codes of Conduct. We support the academy in its undertaking and encourage PEAK Football to take necessary disciplinary action including the suspension and banning of any players, parents, spectators or officials for repeated and serious breaches of these Codes of Conduct.
